“Smartwatch” is doing a lot of work in the phrase “best smartwatch for health tracking,” and a lot of articles on this topic quietly cheat the definition. They include fitness bands, smart rings, even chest straps. We are not going to do that. For this list a smartwatch means: a wrist device with a real touchscreen, third-party apps, on-wrist notifications, and a watch face you would actually wear in public. That rules out the Fitbit Charge line (a tracker), the Oura Ring (a ring), and the Whoop (a screenless band). What is left is a smaller field than the marketing suggests, and the honest answer is that it splits along ecosystem lines before it splits along sensor lines. The Apple Watch is the best health-tracking smartwatch on the market if you have an iPhone, and it is not available to you at all if you don’t. Samsung’s best features require a Samsung phone, not just any Android. Garmin and Withings are the only two on this list that do not care which phone you carry. We will walk through all five.

How we evaluated these

Five things, weighted roughly in this order:

  1. Sensor stack and what the regulator says about it. An “ECG feature” is not the same as an FDA-cleared single-lead ECG with an atrial-fibrillation indication; the second thing has a 510(k) number you can look up. We care about the second thing.
  2. Ecosystem fit. A watch that works half as well on the wrong phone is not the same product, and we say so.
  3. Real-world battery life with the health features on. Not the marketing number with everything turned off.
  4. Sleep tracking quality. Heart rate, HRV, skin temperature, respiration, ideally with stage detection that does not embarrass itself against a Zeo or PSG. (Almost no consumer device clears the PSG bar; we say so when we mean it.)
  5. What happens if you try to leave. Data export, account portability, whether the watch keeps working if the cloud goes dark. We have a longer view on this after watching Intel turn off the Basis cloud in 2016.

Subscription pricing is a tiebreaker. None of these five paywall basic health metrics, which is the bar; some try harder than others to upsell you on coaching content.

1. Apple Watch Series 9 — best if you have an iPhone

  • FDA-cleared single-lead ECG (510(k) K181150) with an atrial-fibrillation indication; Apple's submission data showed 98.3% sensitivity and 99.6% specificity against a 12-lead reference.
  • AFib History feature (cleared as part of K213971) tracks AFib burden over time — the first consumer wearable allowed to make that quantitative claim.
  • Irregular Rhythm Notification runs continuously in the background; clearance K203501. Lower-bar than ECG but it is what catches things in the first place.
  • Skin temperature sensor on the underside (Series 8 and later) tracks overnight baseline drift, used for cycle estimates and illness flagging.
  • Fall detection and Crash Detection are validated to a level no other watch on this list matches; the latter is genuinely effective and the false-positive rate on Series 9 has dropped substantially from the original 2022 implementation.
  • Tight integration with the Health app means data is in one place, exports as XML, and is portable between iPhones via iCloud.

Cons

  • Eighteen-hour battery in Apple's own claim; we get a real day plus overnight if we charge for 30 minutes in the shower. Sleep tracking and daily charging are in tension and Apple has never solved this.
  • Blood oxygen sensor is present in hardware but disabled on new US units sold since January 2024 because of the Masimo patent dispute; older watches and international units still have it active. Check your serial number before buying used.
  • ECG and AFib History require an iPhone, not just iOS. An iPad cannot serve as a primary pairing device, and an Android phone will not pair at all.
  • Apple Health's export format is workable XML but the ecosystem actively discourages third-party tools; Garmin and Withings export cleaner data with less friction.
Best for: iPhone users who want every available FDA-cleared health feature and accept charging the watch every day.

The Series 9 is the easy recommendation if your phone is an iPhone, because no other watch packages this many FDA-cleared health features in one device. The ECG and AFib History pair are the only consumer system we know of that can both screen for atrial fibrillation and quantify how much of it you are experiencing — Apple specifically applied for and received clearance for the second claim, and it is the kind of detail that distinguishes a real medical-adjacent feature from a marketing one. Battery life is the part we cannot defend. Eighteen hours is generous for sleep tracking, and the workaround — charging in the shower, charging while you cook breakfast — is a daily ritual the rest of this list mostly avoids. Apple’s bet has been that the watch is so useful that you will tolerate the charging cycle. They are mostly right, but it is worth saying out loud.

The blood-oxygen situation deserves a callout. After Masimo won an ITC ruling in late 2023, Apple disabled the SpO2 feature on Series 9 and Ultra 2 units sold in the US starting January 2024. Watches sold before that date, or sold outside the US, still measure SpO2. 2. Apple Watch SE (2nd Gen) — best Apple budget pick

Cons

  • No ECG, no AFib History, no Irregular Rhythm Notification (the SE explicitly omits the electrical heart sensor).
  • No skin temperature sensor.
  • No SpO2 sensor (which on the Series 9 is disabled anyway in the US, but is hardware-present).
  • Always-on display is not available; you raise your wrist or tap to wake. Minor irritation for some, fine for others.
  • Aluminum case only; no stainless or titanium options.
Best for: iPhone users who want fall detection, crash detection, and Apple's heart-rate monitoring without paying for ECG and temperature.

If we were buying an Apple Watch for someone who is healthy, not specifically worried about a cardiac history, and not interested in skin temperature, we would buy them the SE without much hesitation. The omissions are real but they cluster at the medical-adjacent end of the feature list. Optical heart rate, workout tracking, fall detection, and the entire watchOS experience are identical. You are paying roughly $150 less for a watch that does 80% of what the Series 9 does on the health-tracking front, which is a defensible trade.

The SE is also the smarter buy if you are giving an Apple Watch to an older parent who wants fall detection and emergency SOS, because those features do not require ECG. The bigger watch upgrade for that user is not the sensor stack, it is cellular — being able to call for help without the phone nearby. The SE supports cellular on the GPS+Cellular SKU.

3. Samsung Galaxy Watch 7 — best for Android (with caveats)

Cons

  • ECG, BIA, and several of the headline health features require Samsung Health on a Samsung Galaxy phone. They do not work on a Pixel or a OnePlus, even though the watch otherwise pairs with any Android device. This is the single biggest gotcha and Samsung does not advertise it prominently.
  • Sleep apnea screening is currently US-cleared; rollouts in other regions follow local regulatory timelines. If you are buying for this feature specifically, confirm availability in your country.
  • Wear OS update support has historically been slow; Samsung has committed to four years of major OS updates on the Watch 7, but the company's track record is mixed.
  • iPhone pairing is not supported. This is an Android-only product.
Best for: Galaxy phone owners who want ECG, body composition estimates, and the experimental sleep apnea feature.

The Galaxy Watch 7 is genuinely impressive on paper and is the right pick for someone already in the Samsung phone ecosystem. The sleep apnea screening feature is the most interesting new health capability shipped by a major smartwatch in 2024-2025 — the De Novo authorization is a higher bar than a 510(k) and applies to a condition that is wildly under-diagnosed in the general population. About 80% of people with moderate-to-severe OSA in the US are believed to be undiagnosed. A consumer wearable that nudges a fraction of those people toward a sleep study is a meaningful intervention even if the device itself does not diagnose anything.

The phone-lock problem is the gotcha. ECG, BIA, blood pressure (in markets where Samsung’s BP feature is approved), and the sleep apnea screen all require Samsung Health, which in turn requires a Galaxy phone for full functionality. We have seen Pixel owners buy this watch expecting Apple-Watch-like parity with their phone and discover that the headline features they paid for are inaccessible. If you are not on a Galaxy, this is the wrong watch, full stop — get a Garmin Venu 3 instead.

4. Garmin Venu 3 — best for athletes who also want a smartwatch

The smartwatch shape for people who actually care about training data. Wear OS apps it is not — Garmin OS it is, and that is fine.

Pros

  • Real-world battery life of 10-14 days in smartwatch mode without GPS workouts, dropping to about 5-7 with daily GPS use. The longest battery of any product on this list outside the ScanWatch.
  • Training metrics inherited from the Forerunner line: VO2max estimate, Training Status, Training Load, Race Predictor, Body Battery. No subscription required for any of it.
  • Sleep tracking with HRV-based recovery scoring is now Garmin's strongest feature; the 2023-2024 firmware updates closed most of the gap with Oura on stage accuracy.
  • AMOLED touchscreen, 41mm and 45mm sizes, looks like a smartwatch rather than a sport watch — important if that is what you wanted.
  • Garmin Connect's data export is best in class. Bulk FIT or CSV download of every metric you have ever recorded, free, one click.
  • Garmin Pay, music storage with offline Spotify, basic third-party app store via Connect IQ.

Cons

  • No ECG. Garmin has consistently chosen not to pursue 510(k) clearance for an ECG feature, and the Venu line reflects that. If ECG is the reason you are buying a smartwatch, this is the wrong watch.
  • No cellular variant. The Venu 3 pairs with a phone over Bluetooth; there is no LTE SKU.
  • Garmin Connect IQ app store is a long way behind watchOS and Wear OS in app variety and quality.
  • Phone notifications work on both iOS and Android but are read-only on iPhone (Apple's restriction, not Garmin's).
Best for: Runners, cyclists, and triathletes who want full Garmin training analytics on a watch they can also wear to a dinner.

The Venu 3 is our pick for the buyer who wants a smartwatch shape but a sport watch brain. Garmin’s training metrics are the deepest in consumer wearables, and on the Venu 3 they sit inside a watch that is small enough and pretty enough that we have worn one to a wedding without anyone commenting. The trade is the app ecosystem: Garmin OS is not Wear OS, and the Connect IQ store is a long way from being able to replace it. If you want Google Maps and the full third-party app catalog on your wrist, you want a different watch.

The no-subscription point is worth dwelling on. Strava charges, Whoop charges, Oura charges, Fitbit Premium charges. Garmin sells you a watch and then leaves you alone. Training Status, Race Predictor, recovery metrics, HRV, sleep stages — all of it is in the free Garmin Connect app. After a decade of watching the wearable industry move toward subscription-everything, that matters more to us each year. Battery life is the other quietly excellent thing. Ten days of real use in smartwatch mode is roughly fifteen times what an Apple Watch manages, and you can sleep-track without choreographing your charging schedule. Garmin’s GPS battery efficiency has also improved enough that a 10-hour ultramarathon does not drain the watch.

5. Withings ScanWatch 2 — best hybrid for low-fuss health tracking

  • FDA-cleared ECG (510(k) K221976) with rhythm notification clearance added by amendment. One of the better-validated consumer ECG systems on the market.
  • TempTech24/7 module measures skin temperature continuously through the night, used for cycle tracking, illness flagging, and base metabolic estimates.
  • Sapphire glass, stainless steel case, analog hands — looks like a watch in a meeting, not a notification device.
  • Excellent data export. Every measurement you have recorded is available as CSV from your Withings account.
  • No subscription required for stages, HRV, ECG, or temperature trends; Withings+ is optional coaching content.

Cons

  • The OLED sub-dial is fine for a heart rate or a notification glance; it is not a smartwatch screen. You cannot scroll a workout playlist or reply to a message.
  • No third-party app store at all. Notifications come through, controls go out, that is it.
  • $349 MSRP is the highest on this list.
  • Pairs with iOS and Android but the ScanWatch experience does not change based on which one you carry — there is no Apple Watch-style deep iOS integration.
Best for: Professionals who want continuous heart monitoring and FDA-cleared ECG without the daily-charge ritual or the look of a smartwatch.

The short version is that it is the watch we recommend for people who do not actually want a smartwatch but do want the health data. It looks like a Swiss-made automatic from across the room. It charges once a month. The OLED is small enough that you cannot doom-scroll on it.

The ECG clearance is real and worth a beat: 510(k) K221976, with an amendment in 2023 adding continuous rhythm notification. That is a higher bar than the static ECG-on-demand feature alone, and it puts the ScanWatch on the small list of consumer devices that can flag potential AFib in the background rather than only during a 30-second hold.

The compromise is the screen. If you want apps, music, payments, and a real notification surface, this is the wrong product — buy an Apple Watch or a Galaxy. If you want the health features and you have made peace with picking up your phone for everything else, the ScanWatch 2 is a quietly excellent product that is much rarer than it should be.

The ecosystem trap is the real decision

Most of the questions we get about this list come down to “I want an Apple Watch but I have an Android” or “I have a Samsung phone but my partner is on iPhone, can we share data?” The answer is almost always: the ecosystem decision is bigger than the watch decision. A few specifics, because the marketing obscures this:

watchOS is fully iPhone-only. There is no Android pairing path, official or otherwise. ECG, AFib History, and the temperature sensor all require iOS. Apple Health data syncs across iPhones and iPads via iCloud but does not export cleanly to anything outside the Apple universe. If you switch to Android, you lose access to historical Apple Health data in any practical sense.

Wear OS is the underlying platform on the Galaxy Watch 7, but Samsung layers Samsung Health on top, and Samsung Health’s most interesting features (ECG, BIA, sleep apnea screening) require a Samsung Galaxy phone. A Pixel can pair with a Galaxy Watch, but the experience is materially worse. The Pixel Watch 3 — which we have not included here because it does not yet ship FDA-cleared ECG with the same range of indications — is the Pixel-native answer, and is improving fast.

Garmin Connect runs on iOS and Android equally well. The watch experience is identical regardless of phone. Garmin has no horse in the phone race, which is part of why we recommend the Venu 3 to anyone whose primary phone is anything other than a Samsung Galaxy or an iPhone.

Withings Health Mate is also phone-agnostic. The ScanWatch 2 pairs with any reasonably modern iOS or Android device and the feature set does not change. Withings’s account model is closer to a fitness data warehouse than to an ecosystem, which fits the watch’s character.

The implication is that if you are deciding between an Apple Watch and a Galaxy Watch on health features, you have already made the bigger decision by picking your phone. We have seen people switch phones to get the Apple Watch they wanted, and at least one go the other way to get BIA and sleep apnea screening on a Galaxy. That is, frankly, a lot of work for a watch.

Medical claims: what these watches can and cannot say

We have been careful in this piece to distinguish “FDA-cleared” features from general wellness ones, because the regulatory line matters and the marketing obscures it.

A 510(k) clearance is a specific permission to make a specific claim — for example, that the Apple Watch’s ECG feature can identify signs of atrial fibrillation in adults 22 and older with no diagnosed history of AFib. The clearance does not authorize a diagnostic claim. The watch can flag a rhythm. A clinician diagnoses the rhythm. The same applies to the Galaxy Watch’s ECG and to the ScanWatch’s. The Galaxy Watch’s sleep apnea feature went through De Novo (DEN230064), a different and somewhat higher-bar pathway because no predicate device existed; the cleared claim is for risk identification, not diagnosis.

Wellness features — Body Battery, Readiness, the AGEs Index, BIA body composition — are explicitly not cleared as medical devices and the manufacturers do not claim they are. They are pattern-recognition layered on sensor data, useful as directional signals, not as numbers to make medical decisions on. We labor this point because the line gets blurred constantly in user reviews and YouTube videos. If a watch makes a quantitative health claim, look up the 510(k) or De Novo number; if it is not there, the claim is wellness-grade. That is fine, as long as you treat it as such.

Who should buy what

Quick decisions, in the order we would actually have them with a friend:

  • iPhone, want the best health watch, do not mind charging daily: Apple Watch Series 9.
  • iPhone, want most of the health features and a much better price: Apple Watch SE 2nd Gen.
  • Samsung Galaxy phone, want the most experimental and most fully integrated Android health watch: Galaxy Watch 7.
  • Any phone, especially Pixel or OnePlus, and you run or cycle: Garmin Venu 3.
  • Any phone, you want the data without looking like you are wearing a smartwatch, and you are tired of charging things: Withings ScanWatch 2.

The Series 9 is the most capable health-tracking smartwatch on the market right now. The ScanWatch 2 is the one we are most likely to recommend to someone who is not specifically asking us, because the calculus of charge-once-a-month plus FDA-cleared ECG plus a watch that does not look like a computer is unusual and durable. The Venu 3 is the underrated one, because it solves the Android-without-Samsung problem better than anything else and does not gate any of its data behind a paywall.

What none of these are is a replacement for medical care. A consumer ECG is a screening tool. A sleep apnea screen is a risk indicator. A skin-temperature trend is a hint. The watches on this list have moved a meaningful number of people toward an actual cardiologist or sleep specialist in the last few years, which is the right outcome. Buying a watch to avoid talking to a doctor is the wrong outcome, and we want to be plain about that.
